You seem like a nice guy so I'll jump in!
I would say mention summer school, because it shows you are serious. If they are good, perfect. If they are not, you can at least argue you still went through with it, and made an effort (you didn't just sit down and do nothing).
I'm not sure about transferring. Is reinstatement on a faculty or a program basis? Because if its the latter, the would recommended something like "if I do not reach the requirements to be reinstated into life science, I would appreciate physical chem or enviro sci." I know people who have moved from those two programs back into life sci in second year.
