Parent Information Form Due August 31st
Parent Information Form Due August 31st
To all first years entering in the 09/10 fall term:
The New Parent Information Form is due on August 31st, 2009. This form is mandatory and should be filled out and submitted it on time. All students (residence & off-campus) must fill this out.
Find it here: Parent Information Form
To clear confusion, it was not implemented last year [08-09] but it is this year.

So... a ton of people are asking me what the point is of this form, what is it for, and why do they need to fill it out ... and what will happen if they don't by the due date? Anybody know? I mean, I didn't have to fill it out last year because they never had it last year.

From the looks of it, it's probably just for reference and to collect alumni info... Or, if you're under 18 in September, McMaster has a number to call.
Since most of the fields do not have to be filled in, I'm going to assume it's just for reference in case something bad happens.

What I'm wondering is, how do you fill this form out if God-forbid, one (or both) parent(s) are deceased?
They didn't provide an option for that.

From what I was told it's for Alumni records. I don't think it's anything really important but it's mandatory for whatever reason. Parents can sign up for a newsletter if they wish?
I don't know what else to say. Maybe alumni can enlighten us.

You were informed of it though. You received it in a package via mail. Also, you were asked to fill it out if you attended Mission to Mac. Read everything you get.

I don't know why we have to fill this out. I'm just filling it out with the bare minimum ie names and home address. Both things that McMaster already has on file
