It's not mandatory, I think it's just in the event of an emergency.

No, it's not your emergency contact information - that is done via MUGSI. The parent information form is completely separate.
It's just to gather demographic information and potentially send your parents some information (nothing personal, so they don't get sent your grades or anything like that). It says at the bottom of the form what it is for.
